PMDD: What It Is, Why It Happens & A Root Cause Approach

It may be Premenstrual Dysphoric Disorder (PMDD), a condition that can deeply impact mental health, relationships, work, and quality of life. Yet so many women are dismissed, told it is normal, or offered little support beyond “just go on the pill”.

What is PMDD?
PMDD (Premenstrual Dysphoric Disorder) is a severe form of PMS that occurs in the luteal phase of the menstrual cycle (after ovulation and before your period). Symptoms often improve or disappear once bleeding begins.
PMDD is NOT “bad PMS”. It is a recognised condition involving a heightened sensitivity to hormonal shifts.
Common symptoms can include:
Severe mood swings
Rage, irritability, or anger
Anxiety or panic attacks
Depression or low mood
Feeling hopeless or overwhelmed
Brain fog or poor concentration
Fatigue and low motivation
Breast tenderness, bloating, headaches, or period pain
Feeling unlike yourself or struggling in relationships
Many women describe feeling like they only have one or two good weeks each month.
What causes PMDD?
Research suggests PMDD is not caused by abnormal hormone levels, but rather by a heightened sensitivity to normal hormonal fluctuations, particularly after ovulation.
That said, there are often contributing factors that can worsen symptoms or make the body less resilient to hormonal shifts.
These may include:
Histamine intolerance or mast cell activation
Histamine and oestrogen have a close relationship. Higher oestrogen levels can increase histamine, while histamine can stimulate more oestrogen production, creating a frustrating cycle.
For some women, this may contribute to:
Anxiety
Migraines or headaches
Skin flare-ups
Sleep issues
Mood changes around ovulation or before a period
Oestrogen dominance or poor hormone clearance
Some women may experience symptoms linked with higher oestrogen relative to progesterone, or difficulty clearing hormones effectively.
Contributing factors may include:
Gut health issues
Constipation
Poor liver detoxification pathways
Chronic stress
Exposure to environmental toxins
Gut health and the microbiome
The gut plays an important role in hormone metabolism. An imbalanced microbiome may affect how oestrogen is processed and cleared from the body.
Digestive symptoms, bloating, irregular bowel movements, or food sensitivities can sometimes be clues worth exploring.
Stress and nervous system load
Chronic stress can significantly impact hormone balance, resilience, and neurotransmitters involved in mood regulation.
Women with PMDD are often already running on empty, making the nervous system more reactive to hormonal changes.
Nutrient deficiencies
Certain nutrients are involved in hormone metabolism and mood support, including:
Magnesium
Vitamin B6
Zinc
Omega-3 fats
Iron (particularly if periods are heavy)
Thyroid dysfunction, inflammation, and blood sugar
Low thyroid function can sometimes overlap with symptoms like fatigue, low mood, brain fog, and heavy periods.
Inflammation and blood sugar imbalances may also worsen mood symptoms, irritability, and energy crashes throughout the luteal phase.
A root cause naturopathic approach to PMDD
There is no one size fits all approach to PMDD.
