Normal Thyroid Blood Tests… But Still Exhausted? Here’s What Might Be Missed

You’re tired all the time.

You’re more foggy than you used to be. Your weight has changed without explanation. Your mood feels off.

So you do the right thing, you go to your GP, get blood tests done…

And you’re told:

“Everything is normal.”

But you don’t feel normal.

This is one of the most common patterns I see in clinic, especially in women trying to keep up with demanding jobs, mental load, and life.

And I want to be very clear here:

If your symptoms are real, your body is communicating something.

Even if your blood test says otherwise.

So what is the thyroid actually doing?

Your thyroid is a small gland in your neck, but it has a huge job.

It helps regulate:

  • Energy and metabolism

  • Brain function and focus

  • Mood and stress tolerance

  • Digestion

  • Menstrual cycles

When it slows down, everything can feel slower, both physically and mentally.

The problem with “normal” thyroid results

Most standard GP testing focuses on one marker:

TSH (Thyroid Stimulating Hormone)

The issue?

TSH is not your actual thyroid hormone.

It’s a signal from your brain telling your thyroid what to do.

So you can have a “normal” TSH and still have symptoms, because it doesn’t show how well your body is actually using thyroid hormone.

It’s a bit like checking the dashboard lights in a car and assuming the engine is running perfectly.

What’s often missed

When symptoms persist, there are other markers that can matter just as much:

  • Free T3 & Free T4: These are your active thyroid hormones, the ones that drive energy, metabolism, and cognition

  • Thyroid antibodies (TPO, TgAb): These can indicate autoimmune activity like Hashimoto’s thyroiditis, even before hormones shift

  • Nutrient status (iron, iodine, selenium, zinc): These are essential for thyroid hormone production and conversion

Why you still feel unwell

Thyroid symptoms don’t always show up neatly in blood work.

Many women are told they’re “in range,” while still experiencing:

  • Fatigue that sleep doesn’t fix

  • Brain fog and slower thinking

  • Constipation or bloating

  • Anxiety or low mood

  • Weight changes that feel out of proportion

  • Feeling cold easily

  • Irregular or heavy cycles

  • Hair thinning or dry skin

If this sounds familiar, you’re not alone.

And no, it’s not “just stress” either.

Why this happens

Thyroid function is influenced by more than just the gland itself.

Common underlying drivers include:

  • Chronic stress and burnout

  • Autoimmune activity

  • Past viral infections

  • Nutrient deficiencies

  • Gut health imbalances

  • Ongoing inflammation

Which is why two people with the same lab results can feel completely different.

The iodine piece most people miss

Iodine is essential for thyroid hormone production.

But deficiency is more common than most people realise, especially with:

  • Low seafood intake

  • Environmental exposure to fluoride and chlorine

  • Processed food diets

This is one of the reasons a deeper functional approach to testing can be so helpful.

What actually helps

If symptoms persist despite “normal” results, the goal isn’t to guess, it’s to look deeper.

A more comprehensive approach may include:

  • Full thyroid panel (not just TSH)

  • Nutrient testing

  • Inflammatory and autoimmune markers

  • Root cause assessment (stress, gut, immune system)

  • Personalised nutrition and herbal support

Because there is rarely just one cause.

The bigger issue

Many women are told:

  • “Your results are fine”

  • “It’s just stress”

  • “Try sleeping more”

While continuing to feel worse.

And over time, they start doubting themselves instead of their results.

You don’t need to feel “sick enough” for your symptoms to matter.

If your body feels off, that’s valid information.

Blood tests are a tool, not the full picture.

And when we zoom out and look at the whole system, things often start to make sense.
